ABSTRACT

ABSTRACT Background: Chagas disease, a zoonosis transmitted mainly by hematophagous insects of the subfamily Triatominae, is caused by Trypanosoma cruzi, classified into six discrete typing units (DTUs: TcI-TcVI and Tcbat). Methods: Insect vectors were collected from 84 human dwellings in the municipality of Santo Domingo Tehuantepec, Oaxaca, Mexico; 4.76% were infested. DTUs were determined using conventional and nested PCR. Results: The infection rate was 43.6%. All insects were infected with TcI while one specimen showed mixed infection with TcII. Conclusions: This is the first report of T. cruzi mixed infection in Triatoma phyllosoma, its main vector in the study region.

ABSTRACT Background: We report the presence of Triatoma longipennis for the first time in two localities in Hidalgo, Mexico. Methods: This study was conducted at Tecozautla municipality, Hidalgo. Collection was performed in April 2022. Results: We collected eight triatomines from Guadalupe: two fourth-instar nymphs, three fifth-instar nymphs, one female, and two males. In San Miguel Caltepantla, a female was collected inside a dwelling. One sample tested positive for Trypanosoma cruzi. Conclusions: These findings suggest the need to investigate the dynamics of this species with respect to the inhabitants of the study area.

Abstract This study aimed to evaluate diagnostic techniques for trypanosomiasis, caused by Trypanosoma vivax, in naturally infected cattle in Minas Gerais, Zona da Mata. The deaths of six lactating cows with similar clinical conditions—characterized by hyporexia, hypogalactia, and recumbency—had been reported from one property. Initially, two animals were examined and diagnosed with trypanosomiasis through identification of the protozoan in a blood smear. After the initial diagnosis, all lactating cows (n=37) on the property were examined, and blood samples were collected for tests including whole blood smear, buffy coat smear, Woo's technique, enzyme-linked immunosorbent assay (ELISA), and polymerase chain reaction (PCR). Woo's test, buffy coat smears, and whole blood smears indicated that 4/37 (10.81%) animals were positive for trypanosomiasis, whereas ELISA and PCR indicated that 33/37 (89.19%) and 27/37 (72.97%) animals, respectively, were positive. The agreement obtained between parasitological techniques was classified as high, while between ELISA and PCR, no agreement. In conclusion, parasitological techniques have a low capacity to identify infected animals in the chronic stage of T. vivax infection. Therefore, techniques such as PCR and/or ELISA should be used to minimize the occurrence of false negatives.


Resumo Este estudo objetiva avaliar as técnicas de diagnóstico da tripanossomíase, causada pelo Trypanosoma vivax, em bovinos naturalmente infectados, em Minas Gerais, Zona da Mata. A morte de seis vacas em lactação com condições clínicas semelhantes - caracterizadas por hiporexia, hipogalaxia e decúbito - foi relatada em uma propriedade. Inicialmente, dois animais foram examinados e diagnosticados com tripanossomíase através da identificação do protozoário em esfregaço sanguíneo. Após o diagnóstico inicial, todas as vacas em lactação (n = 37) na propriedade foram examinadas, e amostras de sangue foram coletadas para testes, incluindo esfregaço de sangue total, esfregaço de capa leucocitária, técnica de Woo, ensaio imunoenzimático (ELISA) e reação em cadeia da polimerase (PCR). O teste de Woo, os esfregaços de capa leucocitária e de sangue total indicaram que 4/37 (10,81%) animais foram positivos para tripanossomíase, enquanto ELISA e PCR indicaram que 33/37 (89,19%) e 27/37 (72,97%) animais, respectivamente, foram positivos. A concordância entre técnicas parasitológicas foi classificada como alta, enquanto entre ELISA e PCR, sem concordância. As técnicas parasitológicas apresentam baixa capacidade para identificar animais infectados na fase crônica da infecção por T. vivax. Dessa forma, técnicas como PCR e/ou ELISA devem ser utilizadas para minimizar a ocorrência de falsos negativos.

ABSTRACT BACKGROUND: Dysphagia is the most frequent digestive symptom in Chagas disease, although other symptoms are reported. These symptoms can be associated with the degree of radiological impairment of the esophagus and the duration of dysphagia. OBJECTIVE: This investigation aimed to assess the symptoms and the time of dysphagia related to the different degrees of megaesophagus in patients with Chagas disease. METHODS: A total of 29 patients aged 48 to 73 years participated in this investigation. All of them had dysphagia and a positive serum result for Chagas disease. They were submitted to the assessment of symptoms and radiological examination of the esophagus to assess the degree of megaesophagus, which ranged from I (mild change) to IV (intense change). Dysphagia was quantified with the Eating Assessment Tool (EAT-10). RESULTS: Twelve (41%) patients had megaesophagus degree I, 9 (31%) had degree II, and 8 (28%) had degrees III (6) and IV (2). The intensity of dysphagia was not related to the result of the radiological examination, with EAT-10 median of 5.5 for the degree I, 9.0 for degree II, and 5.5 for degrees III and IV (P>0.25). Choking (14%), regurgitation (21%), voice complaint (21%), weight loss (17%), and odynophagia (17%) were not related to the degree of megaesophagus. Voice changes and odynophagia were related to the patients' time of dysphagia. Likewise, the frequency of symptoms and EAT-10 values were related to the duration of dysphagia. CONCLUSION: The longer the patient had dysphagia, the more frequent were the symptoms reported by the patients. There was no relationship between the degrees of megaesophagus and the symptoms and intensity of dysphagia.


RESUMO CONTEXTO: Disfagia é o mais frequente sintoma digestivo da doença de Chagas; entretanto, outros sintomas podem ser referidos. Esses sintomas podem ser associados ao grau de comprometimento radiológico do esôfago e à duração da disfagia. OBJETIVO: Avaliar os sintomas e o tempo de disfagia relacionados com os diferentes graus de megaesôfago em pacientes com doença de Chagas. MÉTODOS: Participaram da investigação 29 pacientes com idades entre 48 e 73 anos, todos com disfagia e teste sorológico positivo para doença de Chagas. Eles foram submetidos à avaliação de sintomas e exame radiológico do esôfago para avaliar o grau de megaesôfago, que variou de I (alteração discreta) a IV (alteração intensa). Disfagia foi quantificada pelo método Eating Assessment Tool (EAT-10). RESULTADOS: Doze (41%) pacientes apresentaram grau I de megaesôfago, 9 (31%) grau II, e 8 (28%) graus III/IV. A intensidade da disfagia não foi relacionada com o resultado do exame radiológico, com a mediana do EAT-10 de 5,5 para o grau I, 9,0 para o grau II, e 5,5 para os graus III/IV (P>0,25). Engasgo (14%), regurgitação (21%), queixa vocal (21%), perda de peso (17%), e odinofagia (17%) não foram relacionados ao grau de megaesôfago. Houve relação entre alteração vocal e odinofagia com o tempo que os pacientes tinham disfagia. Houve relação entre frequência de sintomas e valores do EAT-10 com a duração da disfagia. CONCLUSÃO: Quanto mais longo o tempo que o paciente tem disfagia maior a frequência de sintomas referidos pelos pacientes. Não há relação entre graus de megaesôfago com os sintomas e a intensidade da disfagia.

ABSTRACT Introduction: About half a million patients in Colombia are currently infected with Trypanosoma cruzi. However, little is known about patients with Chagas disease and anatomical defects such as dextrocardia. Case presentation: A 52-year-old male patient with a 4-year history of dyspnea, chest pain, lower limb edema and syncope (requiring hospitalization), arrhythmias and dextrocardia, underwent serological tests for T. cruzi that were positive. A literature review was conducted to find case reports of patients with dextrocardia or situs inversus and Chagas disease in order to determine the proper treatment. Conclusion: Cases of patients with dextrocardia and Chagas disease are rare. Besides the reported case, only three other cases were found in the literature, which were relatively similar, although they could be considered more severe. According to the findings, the use of etiological treatment is acceptable in patients with coronary anatomic abnormalities and T. cruzi infection. The present case draws attention to the importance of adequately approaching and monitoring this type of patient.


RESUMEN Introducción. En la actualidad, en Colombia hay aproximadamente medio millón de personas infectadas con Trypanosoma cruzi; sin embargo, no hay mucha información sobre pacientes que viven con enfermedad de Chagas y anomalías anatómicas como la dextrocardia. Presentación del caso. Paciente masculino de 52 años con cuadro clínico de aproximadamente cuatro años de evolución consistente en disnea, dolor torácico, edema de extremidades inferiores, síncope (que requirió hospitalización), arritmias y dextrocardia, a quien se le practicaron pruebas serológicas para T. cruzi que resultaron positivas. Con el fin de establecer el tratamiento adecuado, se realizó una revisión de la literatura buscando reportes de casos de pacientes con dextrocardia o situs inversus y enfermedad de Chagas. Conclusión. Los casos de pacientes con dextrocardia y enfermedad de Chagas son poco frecuentes: además del caso reportado, en la literatura solo se encontraron tres reportes adicionales, los cuales fueron relativamente similares, aunque podrían considerarse más severos. Según los hallazgos, el uso de tratamiento etiológico es adecuado en pacientes con anormalidades anatómicas cardiovasculares e infección por T. cruzi. El presente caso llama la atención sobre la importancia de tener un enfoque y seguimiento adecuados en este tipo de pacientes.

INTRODUCTION: Chagas disease (CD) is a disease caused by the protozoan flagellates of the Kinetoplastid order Trypanosoma cruzi. Approximately 8,000,000 people are infected worldwide, mainly in Latin America, causing disabilities and more than 10,000 deaths per year OBJECTIVE: This study aimed to describe the epidemiological panorama of CD in the Western Brazilian Amazon from 2007 to 2018. METHODS: In this ecological study, secondary data regarding the confirmed cases of T. cruzi infection in the states of Acre, Amazonas, Rondônia, and Roraima were collected from the Single Health System Notification Information System of the Department of Informatics of the Single Health System and were analyzed. The data were used to characterize the epidemiological profile of T. cruzi infection and to determine the frequency of infection in Western Amazonia. RESULTS: A total of 184 cases of CD were reported in Western Amazonia, and the highest number of cases was reported in the states of Amazonas and Acre. CONCLUSION: The epidemiological panorama of the Western Brazilian Amazon from 2007 to 2018 includes a greater number of cases of T. cruzi infection in men aged 20-39 years and those living in rural areas. Oral transmission was prevalent in the region during the study, and the highest number of cases was reported in the months of April and December. Epidemiological data are an important resource for understanding the dynamics of CD and the main aspects related to the health-disease process.


INTRODUÇÃO: A doença de Chagas (DC) é uma enfermidade causada pelo protozoário flagelado da ordem Kinetoplastida denominado Trypanosoma cruzi. Estima-se que oito milhões de pessoas estejam infectadas em todo o mundo, principalmente na América Latina, causando incapacidades e mais de dez mil mortes por ano. OBJETIVO: Descrever o panorama epidemiológico da doença de Chagas na Amazônia Ocidental brasileira no período de 2007 a 2018. MÉTODO: Trata-se de um estudo ecológico e com coleta e análise de dados referentes aos casos confirmados de infecção por T. cruzi nos estados do Acre, Amazonas, Rondônia e Roraima, por meio de fontes secundárias oriundos do Sistema de Informação de Agravos de Notificação do Sistema Único de Saúde (SINAN) do Departamento de Informática do Sistema Único de Saúde (DATASUS). Os dados foram utilizados para caracterizar o perfil epidemiológico dos infectados por T. cruzi e determinar a frequência da infecção na Amazônia Ocidental RESULTADOS: Houve a notificação de 184 casos de doença de Chagas na Amazônia Ocidental com mais registros nos estados do Amazonas e Acre. CONCLUSÃO: O panorama epidemiológico da Amazônia Ocidental Brasileira no período de 2007 a 2018, compreende uma maior quantidade de casos em indivíduos do sexo masculino, na faixa etária dos 20-39 anos, e provenientes de zona rural. A forma de contágio prevalente na região durante o estudo foi a oral e a maior sazonalidade compreendeu os meses de abril e dezembro. Dados epidemiológicos são um importante recurso para a compreensão da dinâmica da DC e os principais aspectos relacionados no processo saúde-doença.

Resumen | Introducción. Belminus ferroae es un triatomino de comportamiento entomófago, sin embargo, puede alimentarse de vertebrados ocasionalmente. No se ha demostrado infección natural por Trypanosoma cruzi en esta especie, como tampoco la metaciclogénesis del parásito. Objetivo. Examinar la metaciclogénesis de T. cruzi en B. ferroae y la capacidad infectiva de las heces o sus contenidos intestinales en roedores. Materiales y métodos. Se analizaron las heces y la orina expulsadas espontáneamente por los insectos o mediante compresión abdominal o extracción del contenido intestinal a los 10, 20, 30, 40, 50 y 60 días. Se cuantificó la carga parasitaria de T. cruzi y sus formas evolutivas se identificaron con tinción de Giemsa. Asimismo, se evaluó en ratones albinos la capacidad infectiva de los tripomastigotes metacíclicos de T. cruzi obtenidos de las heces o contenidos intestinales de los especímenes infectados. Resultados. El análisis parasitológico reveló tres (15 %) insectos infectados con T.cruzi a los 30 (n=1), 40 (n=1) y 50 (n=1) días después de la infección con cargas parasitarias de hasta 1,62 x 105 tripanosomas/mm3 y porcentajes de metaciclogénesis entre el 3,5 y el 6,78 %. Conclusiones. Se demuestra por primera vez, en una especie del género Belminus, la metaciclogenésis de T. cruzi en condiciones de laboratorio y la capacidad infectiva de las heces para un huésped vertebrado.


Abstract | Introduction: Belminus ferroae is a triatominae with entomophagous behavior. However, it may occasionally feed on vertebrates. Currently, there is no evidence of natural infection with Trypanosoma cruzi or the occurrence of metacyclogenesis in this species. Objective: To test T. cruzi metacyclogenesis in B. ferroae and the infectivity of their feces or intestinal contents in rodents under laboratory conditions. Materials and methods: Twenty nymphs of B. ferroae were infected with an autochthonous strain of T. cruzi (M/HOM/VE/09/P6). Fecal and urine samples were collected from spontaneous droppings or by compressing the bugs' abdomens and, eventually, by removing their gut contents, and then examined at 10, 20, 30, 40, 50, and 60 days. We quantified T. cruzi parasitic load, as well as the evolutionary forms in feces, urine, and intestinal contents by Giemsa staining. Similarly, we evaluated the infectivity of T. cruzi metacyclic trypomastigotes in albino mice. Results: The parasitological analysis showed three insects (15%) infected with T. cruzi at 30 (n=1), 40 (n=1), and 50 (n=1) days post-infection. We observed parasitic loads of up to 1.62 x 105 trypanosomes/mm3 and metacyclogenesis percentages between 3.5% and 6.78%. Conclusions: This is the first time that T. cruzi metacyclogenesis is reported in a species of the genus Belminus under laboratory conditions and the infectivity of Belminus' feces is demonstrated on a vertebrate host.

Abstract Objectives: The main objective is to determine the prevalence of American trypanosomiasis in patients with dilated cardiomyopathy in a tertiary hospital in western Mexico. Methods: From January 1991 to February 2016, 387 consecutive patients with a confirmed diagnosis of dilated cardiomyopathy were included in the study. Cases with ventricular dilatation secondary to ischemic heart disease, valvular heart disease, hypertension, lung disease, pericardial disease, or congenital heart disease were excluded from the study. Diagnosis was made detecting antibodies against Trypanosoma cruzi with two different methods or parasite in blood. Results: Were included 387 patients with dilated cardiomyopathy, Chagas cardiomyopathy was confirmed in 6.9%, two patients in the acute phase (in one, suspected transfusion transmission was detected). Most patients were born in rural areas. About 96.2% showed congestive heart failure, only one patient with apical left ventricular aneurysm manifested palpitations. About 66% with right bundle branch block, left anterior fascicular block, or the association of both, in 14.8%, non-sustained ventricular tachycardia was found. Conclusions: Chagas cardiomyopathy is common in México, mainly in people who were born or lived during childhood in rural areas. It is a common cause of heart failure. Chagas’ heart disease should be suspected in patients receiving a blood transfusion, even without another epidemiological history.


Resumen Objetivo: El objetivo principal del estudio es conocer la prevalencia de tripanosomiasis americana en pacientes con cardiomiopatía dilatada, en un hospital de concentración en el occidente de México. Métodos: Desde enero de 1991 a febrero de 2016 se incluyeron 387 pacientes consecutivos con diagnóstico de cardiomiopatía dilatada, se excluyeron los casos con dilatación ventricular secundaria a cardiopatía isquémica, valvulopatías, hipertensión arterial sistémica, enfermedad pulmonar, enfermedad pericárdica o cardiopatías congénitas. El diagnóstico se realizó mediante la detección de anticuerpos anti-tripanosoma cruzi con 2 métodos positivos diferentes o con la detección del parásito en sangre. Resultados: Se incluyeron 387 paciente con cardiomiopatía dilatada, en el 6.9% se confirmó cardiopatía chagásica; dos pacientes en fase aguda (uno con sospecha de transmisión transfusional). La mayoría de los pacientes provenían de zonas rurales. El 96.2% de los casos presentó insuficiencia cardiaca congestiva, un paciente con aneurisma apical del ventrículo izquierdo solo manifestó palpitaciones. El 66% presentó bloqueo de la rama derecha del haz de His, hemibloqueo anterior izquierdo o la asociación de ambos, en el 14.8% se encontró taquicardia ventricular no sostenida. Conclusiones: La cardiopatía chagásica es frecuente en nuestro medio, principalmente en personas que nacieron o vivieron durante la infancia en áreas rurales. Es causa común de insuficiencia cardiaca. La cardiomiopatía chagásica debe sospecharse en pacientes que reciben transfusión sanguínea, incluso sin otros antecedentes epidemiológicos

ABSTRACT

Chagas disease persists as one of the most important, and yet most neglected, diseases in the world, and several changes in its epidemiological aspects have been recorded since its discovery. Currently, some of the most relevant changes are related to: (i) the reduction in the incidence of the endemic due to the control of the most important vectors, Triatoma infestans and Rhodnius prolixus, in many countries; (ii) the migration of human populations spreading cases of the disease throughout the world, from endemic to non-endemic areas, transforming Chagas disease into a global threat; and (iii) new acute cases and deaths caused by oral transmission, especially in the north of Brazil. Despite the reduction in the number of cases, new challenges need to be responded to, including monitoring and control activities aiming to prevent house infestation by the secondary vectors from occurring. In 1979, Lent & Wygodzinsky(1) published the most complete review of the subfamily Triatominae, encompassing 111 recognised species in the taxon. Forty-two years later, 46 new species and one subspecies have been described or revalidated. Here we summarise the new species and contextualise them regarding their ecology, epidemiologic importance, and the obstacles they pose to the control of Chagas disease around the world.

ABSTRACT: The prevalence of trypanosomiasis in sheep is of worldwide concern. It is a hemoparasitic disease caused by the protozoan Trypanosoma (T) spp., andthe biological cycle of transmission involves susceptible hosts and certain hematophagous flies, such as Tabanids and Stomoxys. The objective of this study was to determine the presence of Trypanosomasp. in sheep (Ovisaries). Through an applied, descriptive-prospective-cross-sectional qualitative study conducted between October 1, 2018, and January 29, 2019, blood samples extracted from the jugular vein were analyzed by the blood smear method using 2 Romanowsky staining techniques (Giemsa and Diff-Quick). Animals studied were between ages of 3 and 10 years, coming from 3 farms in the city of Colimes (Ecuador). Of 100 sampled and processed animals, 2 (2%) were positive for Trypanosoma sp., constituting the first report of this hemoparasite in sheep in Ecuador, and 1 case of Babesia spp. (1%) and 4 cases of Anaplasma marginale (4%) were also identified. However, none of these cases presented symptomatology of any hemotropic-parasitic disease. Subsequently, the positive animals were dewormed.The presence of these parasites wasconfirmedat the study site and,thus, could become a serious animal health problem.


RESUMO: A tripanossomíase em ovelhas é uma hemoparasitose globalmente prevalente causada pelo protozoário chamado Trypanosoma (T) sp. O ciclo de vida de transmissão envolve hospedeiros suscetíveis e alguns dípteros hematófagos como Tabanids e Stomoxys. O objetivo destetrabalho foi determinar a presença de Trypanosoma sp. em ovelhas (Ovisaries). Através de um estudo aplicado, com abordagem qualitativa, do tipo descritivo prospectivo-transversal, realizado entre o dia primeiro de outubro de 2018 e 29 de janeiro de 2019, foram analisadas amostras de sangue extraídas da veía jugular pelo método do esfregaço de sangue, utilizando duas técnicas de coloração Romanowski (Giensa e Diff-Quick). Os animais estudados tinham entre três e dez anos de idade, de três fazendas no municipio de Colimes (Equador). Dos 100 animais amostrados e processados, dois casos (2%) foram positivos para Trypanosoma sp., constituindo o primeiro relato deste hemoparasita em ovelhas no Equador. Além disso, um caso de Babesia sp. (1%) e quatro casos de Anaplasma marginale (4%); entretanto, em nenhum desses casos houve sintomas de qualquer doença hemotropicoparasitária. Animais positivos foram posteriormente desparasitados. A presença desses parasitas foi confirmada no local do estudo e, portanto, pode se tornar um sério problema de saúde animal.

Abstract Trypanosomiasis, caused by Trypanosoma vivax, is responsible for great economic losses among livestock in Africa and South America. During the life cycle of these parasites, they may present different morphological, metabolic and physiological characteristics depending on the interactions that are encountered at each point of their life cycle. Although T. vivax is frequently reported in the circulation of its mammalian hosts, it has the ability to migrate to the tissues of these individuals. However, this characteristic is poorly understood. In this context, we aimed to investigate the presence of T. vivax and the changes caused in different tissues of experimentally infected goats. Despite the animals were not perfused before tissues collection, using different approaches, we demonstrated its presence in different samples, including in the adipose tissue and skin of infected animals. In addition, a mononuclear inflammatory reaction, mostly characterized by an infiltrate of lymphocytes, plasma cells and macrophages were observed. The results highlight the possibility that, like other trypanosomatids, T. vivax may use these tissues during its life cycle. Future studies aiming to elucidate the length of time for which T. vivax remains active in these sites, and whether it uses these sites as a refuge from trypanocidal drugs, and whether it is capable of recolonizing the blood circulation, are much needed.


Resumo A tripanossomíase, causada por Trypanosoma vivax, é responsável por grandes perdas econômicas na bovinocultura da África e da América do Sul. Durante seu ciclo de vida, o parasita pode apresentar diferentes características morfológicas, metabólicas e fisiológicas em função das interações que ele encontra em cada ponto do seu ciclo. Embora o T. vivax seja reportado, frequentemente, na circulação dos seus hospedeiros mamíferos, o protozoário tem a capacidade de migrar para os tecidos desses indivíduos. Entretanto, essa característica é pobremente conhecida. Neste contexto, o objetivo foi verificar a presença, assim como as alterações causadas pelo T. vivax nos diferentes tecidos de caprinos experimentalmente infectados. Apesar dos animais não terem sido perfundidos antes da coleta dos tecidos, utilizando-se diferentes abordagens, foi evidenciada a presença do T. vivax em diferentes amostras teciduais, incluindo no tecido adiposo e pele dos animais infectados. Além disso, foi observada reação inflamatória mononuclear, caracterizada majoritariamente por infiltrado de linfócitos, plasmócitos e macrófagos. Os resultados evidenciam a possibilidade de que, assim como outros tripanossomatídeos, T. vivax pode usar esses tecidos durante o seu ciclo de vida. São necessários futuros estudos, objetivando elucidar o período em que o T. vivax permanece ativo nesses sítios, se ele utiliza esses locais como refúgio das drogas tripanocidas, e se ele é capaz de recolonizar a circulação sanguínea.

Objective To investigate the prognosis of two rare imported patients with human African trypanosomias (HAT) after treatment in a follow-up study, and to evaluate the therapeutic efficacy, so as to provide insights into the treatment of imported HAT patients. Methods The white blood cells in cerebrospinal fluid samples and the trypomastigotes in cerebrospinal fluid and blood samples were monitored in an imported case with Trypanosoma brucei rhodesiense infection 1, 3, 11 and 25 months post-treatment and in an imported case with T. brucei gambiense infection 1, 3, 8 and 12 months post-treatment to evaluate the therapeutic efficacy and prognosis. Results There were 1, 1, 4 and 2 white blood cells in per μL of cerebrospinal fluid in the case with T. brucei rhodesiense infection 1, 3, 11 and 25 months post-treatment, and there were 3, 6, 4 and 3 white blood cells in per μL of cerebrospinal fluid in the case with T. brucei gambiense infection 1, 3, 8 and 12 months post-treatment. In addition, no trypomastigotes were identified in the cerebrospinal fluid or blood samples of either case with T. brucei rhodesiense or T. brucei gambiense infection. Conclusion Following standardized treatment, two imported cases with human African trypanosomiasis cases recover satisfactorily, without any signs of relapse.

ABSTRACT

Abstract Reactivation of chronic Trypanosoma cruzi infection in solid organ transplant recipients (SOTRs) has been reported. The patient presented with a 2-week history of two painful erythematous, infiltrated plaques with central ulceration and necrotic crust on the left thigh. She had a history of chronic indeterminate Chagas disease (CD) and had received a kidney transplant before 2 months. Skin biopsies revealed lobular panniculitis with intracellular amastigote forms of T. cruzi. The patient was diagnosed with CD reactivation. Treatment with benznidazole significantly improved her condition. CD reactivation should be suspected in SOTRs living in endemic areas with clinical polymorphism of skin lesions.

ABSTRACT

This study was aimed at determining the effect of leaves of Senna alata extract on biochemical indices of Wistar rats infected with Trypanosoma brucei brucei . Phytochemical screening revealed the absence of steroids in all extracts, absence of saponins in chloroform extracts and the presence of free anthraquinones only in chloroform extract. Post - infection treatment of animals stirred the emergence of parasitaemia by Day 3. Only animals receiving 200 mg/kg b.wt. of chloroform extract survived by day 16. A significant (P<0.05) decrease in ALT for groups receiving methanol (400 mg/kg b.wt.), chloroform and aqueous extracts and significant (P<0.05) increase in unconjugated bilir ubin in the group receiving methanol extract (200 mg/kg b.wt.) compared to infected not treated rats. Significant (P<0.05) decrease in potassium concentration in groups receiving methanol and chloroform, and a significant (P<0.05) increase in sodium concen tration in the group receiving 400 mg/kg b.wt. of aqueous extract compared to the infected not treated rats. These results thereby demonstrate the ameliorative potential of Senna alata leaves against T. brucei brucei .

Abstract This study investigated the seropositivity for five different tick-borne agents, namely Anaplasma marginale, Babesia bovis, Babesia bigemina, Coxiella burnetii, Anaplasma phagocytophilum, and Trypanosoma vivax in beef cattle in the Brazilian Pantanal. The serum samples collected from animals (200 cows; 200 calves) were used in indirect enzyme-linked immunosorbent assays (iELISA) to detect IgG antibodies against A. marginale, B. bovis, B. bigemina, and T. vivax, and Indirect Fluorescent Antibody Test (IFAT) for detecting IgG antibodies against C. burnetii and A. phagocytophilum. No correlation was observed between seropositivity for C. burnetii and A. phagocytophilum with other agents whereas moderate correlation was observed for A. marginalexB. bigemina x B. bovis. Cows were more seropositive for T. vivax whereas calves were more seropositive for B. bovis and B. bigemina. The highest number of seropositive animals by a single agent was observed for T. vivax (15.2%). Co-seropositivity for T. vivax + A. marginale was higher in cows (25.5%) and for T. vivax + B. bovis + B. bigemina + A. marginale was higher in calves (57.5%). The high seropositivity correlation for A. marginale x B. bovis x B. bigemina is probably due to the presence of the tick biological vector, Rhipicephalus microplus, in the studied farms. Common transmission pathways, mediated by hematophagous dipterans and fomites, may explain the high co-seropositivity of cows for A. marginale and T. vivax. Low seropositivity to C. burnetii is probably due to the type of breeding system employed (extensive). Seropositivity for A. phagocytophilum in only one animal suggests the occurrence of a cross-serological reaction with another agent of the genus Anaplasma.


Resumo Este estudo teve como objetivo determinar a co-soropositividade para agentes transmitidos por carrapatos, como Anaplasma marginale, Babesia bovis, Babesia bigemina, Coxiella burnetii, Anaplasma phagocytophilum, e Trypanosoma vivax em bovinos de corte do Pantanal Brasileiro. Amostras de soro foram colhidas de 400 animais (200 vacas; 200 bezerros) e submetidas a Ensaios Imunoenzimáticos Indiretos (iELISA) para detecção de anticorpos IgG anti- A. marginale, anti- B. bovis, anti- B. bigemina e anti- T. vivax, e à Reação de Imunofluorescência Indireta (RIFI) para detecção de anticorpos IgG anti -C. burnetii e anti- A. phagocytophilum. Ausência de correlação foi vista entre os animais soropositivos para C. burnetii e A. phagocytophilum com os outros agentes e correlação moderada ocorreu entre A. marginale x B. bigemina x B. bovis. Vacas foram mais soropositivas que bezerros para T. vivax e bezerros mais soropositivos que vacas para B. bovis e B. bigemina. Maior número de animais soropositivos para um único agente foi visto para T. vivax (15,2%). Vacas demonstraram maior co-soropositividade para T. vivax + A. marginale (25,5%) e bezerros para T. vivax + B. bovis + B. bigemina + A. marginale (57,5%). A alta correlação entre a soropositividade para A. marginale x B. bovis x B. bigemina é provavelmente devida à presença do vetor biológico, o carrapato Rhipicephalus microplus, nas fazendas estudadas. As vias de transmissão comuns, mediadas por dípteros hematófagos e fômites, podem explicar a alta co-soropositividade das vacas para A. marginale e T. vivax. A baixa soropositividade para C. burnetii é provavelmente devida ao tipo de sistema de criação empregado (extenso). A soropositividade para A. phagocytophilum em apenas um animal sugere a ocorrência de reação sorológica cruzada com outro agente do gênero Anaplasma.

Resumen Introducción: Las zoonosis son enfermedades o infecciones causadas por todo tipo de agentes etiológicos transmisibles desde animales vertebrados a humanos. Durante las últimas décadas, el riesgo para la salud ocasionado por diferentes zoonosis, ha sido generado por la distribución natural de los distintos agentes etiológicos y por la emergencia y reemergencia de estas enfermedades. Objetivo: Estudiar la distribución del riesgo de mortalidad de las cuatro principales zoonosis en Chile continental, basados en datos nacionales de mortalidad, con el objetivo de visualizar geográficamente donde focalizar los esfuerzos de control de estas enfermedades. Metodología: Se estima el riesgo relativo de las principales zoonosis en Chile, mediante estadística Bayesiana. Resultados: Se obtuvo la distribución de las cuatro principales zoonosis de Chile. Discusión/Conclusión: Se obtuvo la distribución de las cuatro principales zoonosis de Chile. Los mapas de riesgo obtenidos muestran una enfermedad parasitaria transmitida por vectores de alto riesgo en el norte, la enfermedad de Chagas; una enfermedad parasitaria de comunidades biológicas en que el hombre es un hospedero accidental, asociada a zonas ganaderas, prevalente en el sur, la hidatidosis; una enfermedad bacteriana transmitida por vertebrados, especialmente por roedores, donde el agua es un vehículo importante, dominante en el centro, la leptospirosis; y una enfermedad viral transmitida por roedores, muy dominante en el sur, la infección por hantavirus.


Background: Zoonoses are infections caused by all types of etiological transmissible agents from vertebrate animals to humans. During the last decades, the risk to health caused by different zoonoses has been a consequence of the natural distribution of the different etiological agents and by the emergence and reemergence of these diseases. Aim: To study the distribution of the risk of mortality of the four main zoonoses in continental Chile, based on national mortality data, with the objective of visualizing geographically where to focus the control efforts of these diseases. Methods: Relative risk was estimated by means of Bayesian Statistics. Results: The distribution in Chile of the main zoonoses was obtained. Discussion/Conclusion: The risk maps obtained show a parasitic disease transmitted by high-risk vectors in the north, Chagas disease; a parasitic disease of biological communities in which man is an accidental host, associated with livestock areas, more prevalent in the south, hydatidosis; a bacterial disease transmitted by vertebrates, especially by rodents, where water is an important vehicle, dominant in the center, leptospirosis; and a viral disease transmitted by rodents, very dominant in the south, the hantavirus infection.

Incidence of fire affected the distribution of tsetse flies, wildlife and prevalence of trypanosome. For the interval of three years (2011-2013) a loss of more than 1million USDassociated with trypanosomiasis is estimated in Simanjiro district. More money is used by pastoralists in treatment of animalsthan in spraying pesticides.Conclusion:It is recommended that, more work to be doneon the impact of ecological factors on trypanosomiasis. Also more effort is required in controlling the disease through collaboration ofdifferent stakeholders such as livestock officers, ecologists, veterinariansand otherrelevant agencies.

Abstract The aim of this study was to determine the frequency and factors associated to Babesia caballi, Theileria equi and Trypanosoma evansi in naturally infected equids from the northeast Brazil. Blood samples from 569 equids (528 horses, 8 mules, and 33 donkeys) were collected and tested for the presence of DNA of each of these protozoan parasites by PCR. Generalized linear models were used to evaluate risk factors associated with the infection. The frequency of T. equi infection was 83.5% (475/569) - 84.3% in horses, and 73.2% in donkeys and mules. The results of the final model indicated that age (senior group) and animal species (mule and donkey group) were protective factors against this pathogen. The frequency of B. caballi infection was 24.3% (138/569) - 23.5% in horses and 34.1% in donkeys and mules. Age (adult and senior group) was considered a protective factor against B. caballi infection whereas animal species (donkey and mule group) were considered a risk factor for the infection. Trypanosoma evansi infection was not detected in any of animals. Our results suggest that equids from the area studied may be infected earlier in life with the etiological agents of equine piroplasmosis and become asymptomatic carriers.


Resumo Este estudo teve como objetivos conhecer a frequência e os fatores associados à infecção por Babesia caballi, Theileria equi e Trypanosoma evansi em equinos naturalmente infectados do nordeste do Brasil. Amostras de sangue de 569 equídeos (528 equinos, 8 muares e 33 asininos) foram coletadas e testadas para a presença do DNA destes parasitos através da PCR. Modelos lineares generalizados foram utilizados na avaliação dos fatores associados às infecções. A frequência de infecção por T. equi foi de 83,5% (475/569) -84,3% (445/528) em eqüinos e 73,2% (30/41) em asininos e muares. Os resultados do modelo final indicam idade (sênior) e espécie (muar e asinina) como possíveis fatores de proteção para este patógeno. A frequência de infecção por B. caballi foi de 24,3% (138/569) - 23,5% (124/528) em eqüinos e 34,1% (14/41) em asininos e muares. As faixas etárias (adulto e sênior) foram identificadas como possíveis fatores de proteção, e a espécie (asinina e muar) como risco para ocorrência de infecção por B. caballi. Infecções por Trypanosoma evansi não foram detectadas. Estes resultados indicam que os equídeos na região estudada se infectam precocemente com agentes da piroplasmose equina tornando-se portadores assintomáticos.

Justificativa e Objetivos: A doença de Chagas (DC), causada pelo Trypanosma cruzi, protozoário flagelado, descoberto a mais de 100 anos, sendo ela difundida nas américas, principalmente na américa do Sul, acometendo milhões de pessoas pelo mundo. É cada vez mais crescente o número de indivíduos acometidos com a DC na região da Amazônia Ocidental e estão relacionados a diversos fatores, porém o consumo de bebida contaminada é a principal parcela de contribuição para a elevação nos registros da doença na região. Este estudo tem como objetivo descrever e analisar o panorama epidemiológico da doença de Chagas no estado do Amazonas de 2004 a 2014. Métodos: Trata-se de um estudo ecológico, onde foram utilizados somente dados secundários obtidos do Sinan referentes aos casos notificados de infecção por T. cruzi em todo o estado do Amazonas. Resultados: Constatou-se que no período de 2004 a 2014 que no estado do Amazonas ocorreram um total de 100 casos de DC, tendo maior ocorrência nos anos de 2007 e 2010, durante os meses de abril e dezembro, com maior frequência da faixa etária de 20-39 e no gênero feminino. Conclusão: Constatou-se a necessidade de implementar um serviço de controle epidemiológico eficaz para monitoramento da via de transmissão DC e de seus vetores, pois trata-se de um problema de saúde pública que pode estar ligada a determinantes sociais e culturais, sendo indicado a promoção de atividades educativas para a conscientização da comunidade a respeito desta enfermidade.(AU)


Background and Objectives: Chagas disease (CD), caused by Trypanosoma cruzi, a flagellate protozoan, was discovered over 100 years ago, spread throughout the Americas, mainly in South America, affecting millions of people around the world. CD affects several people in the Western Region of Amazonia, with increasingly frequent numbers due to several factors that contribute to the increase of cases in the region. but the consumption of contaminated drink is the main way of transmission of the disease in the region. This study aims to describe and analyze the epidemiological panorama of Chagas disease in the state of Amazonas, from 2004 to 2014. Methods: This is an ecological study, where only secondary data obtained from Sinan referring to reported cases of T. cruzi infection in the entire state of Amazonas were used. Results: a total of 100 cases of CD were reported in the state of Amazonas, from 2004 to 2014, , with a higher occurrence in 2007 and 2010, during April and December, with a higher frequency in the 20-39 age group and in women. Conclusion: the need to implement an effective epidemiological control service to monitor the CD transmission route and its vectors was identified, because it is a public health problem that may be linked to social and cultural determinants. The promotion of educational activities to raise awareness of the community of this disease.(AU)


Justificación y Objetivos: La enfermedad de Chagas (EC), causada por Trypanosoma cruzi, protozoario flagelado, descubierto hace más de 100 años, siendo difundida principalmente en América del Sur, afectando a millones de personas por el mundo. EC está afectando varias personas en la Amazonía Occidental con cifras cada vez más frecuentes debido a varios factores que contribuyen a la elevación en los casos en la región, uno de los principales es la forma oral. Este estudio analizó el panorama epidemiológico de la EC en el estado de Amazonas, de 2004-2014. Métodos: Se trata de un estudio poblacional, retrospectivo y descriptivo ejecutando colecta y análisis de datos referentes a los casos relatados de infección por T. cruzi en, el estado de Amazonas, datos secundarios provenientes del Sistema de Información de Agravios de Notificación del Sistema Único de Salud, través del Banco de Datos del Sistema Único de Salud. Resultados: Se constató que de 2004-2014 en el estado de Amazonas ocurrieron un total de 100 casos de EC, teniendo mayor ocurrencia en los años de 2007-2010, durante los meses de abril y diciembre, con mayor frecuencia del grupo de edad de 20-39 y en género femenino. Conclusiones: Se constató la necesidad de implementar un servicio de control epidemiológico eficaz para monitoreo de la vía de transmisión EC y de sus vectores, pues se trata de un problema de salud pública que puede estar vinculada a determinantes sociales y culturales, promoción de actividades educativas para la concientización de la comunidad acerca de esta enfermedad.(AU)

Abstract INTRODUCTION: Trypanosoma cruzi is the etiological agent of Chagas disease (CD), a zoonotic infection transmitted by triatomine bug vectors to human beings. Although the story of this parasitic infection was born in Brazil and here this has made major step forward information, the same cannot be said about the actual distribution of the triatomine vector in several areas of this country. The aim of this study was to assess the occurrence of triatomine species in an endemic region for CD in Northeastern Brazil. METHODS: A retrospective study was performed using data obtained from 2008 to 2017. All information was provided by the V Gerência Regional de Saúde of the state of Pernambuco. The spatial distribution of triatomine species was analyzed by drawing a map using the Quantum geographic information system. RESULTS: A total of 4,694 triatomine specimens (469.4 ± 221.2 per year) were collected during the period 2008-2017, with 94.5% (4,434/4,694) at the intradomicile and 5.5% (260/4,694) at peridomicile environment. Of all arthropods collected, 92.5% (4,340/4,694) and 7.5% (354/4,694) were adults and nymphs, respectively. The species most frequently detected were Panstrongylus lutzi (30.36%), Triatoma brasiliensis (26.12%), Triatoma pseudomaculata (22.43%), and Panstrongylus megistus (20.54%). CONCLUSIONS: These data contribute to a better understanding of the distribution of T. cruzi infection in the Northeastern region of Brazil. Preventive measures based on vector control should be implemented in the study area in order to reduce the burden this neglected tropical disease.
